The patient presented with swelling and oozing of yellow fluid around the face and eyes, accompanied by itching and burning pain. The patient stated that due to freckles and brown spots on her face, she had seen an advertisement on TikTok a few days prior and ordered a skin regeneration combo consisting of a skin regeneration essence and a facial cleansing powder. The combo was advertised as removing freckles, dark spots, uneven skin tone, regenerating new, healthy skin, increasing collagen production, and rejuvenating the skin… for 373,000 VND. The shop employee instructed her to apply the essence to her face three times a day (morning, noon, and evening). Each time she applied it, her skin felt slightly warm and reddened.
“After using it for 3 consecutive days, my face swelled up, became hot, red, burning, itchy, painful, scabbed over, and oozed fluid. Both my eyes were so swollen I couldn't see. I contacted the shop and they instructed me to buy unsweetened fresh milk to mix with the facial cleansing powder in the skin regeneration combo to wash my face, but the condition didn't improve; it got worse. My skin scabbed over, oozed fluid, and felt painful, burning, and itchy… I took pictures and showed them to the shop, and they said it was good, that my skin was undergoing a strong detoxification process…,” the patient shared.
Recognizing that the patient's condition showed signs of allergic contact dermatitis accompanied by a secondary bacterial infection, doctors at the Ho Chi Minh City Dermatology Hospital admitted the patient for inpatient treatment. Dr. Pham Ngoc Tram, Specialist in Dermatology, Clinical Department 1, Ho Chi Minh City Dermatology Hospital, stated that the patient received antibiotic, anti-inflammatory, anti-itch, and local care treatment. After about a week of treatment, the skin swelling and redness decreased, the pustules dried up and scabbed over, and the patient was discharged.
According to MSc-Dr. Le Thao Hien, Department of Aesthetic Dermatology, Ho Chi Minh City Dermatology Hospital, the main cause of the infected and swollen dermatitis in the patient was the use of a skin peeling product with unclear active ingredients and concentrations, resulting in severe skin peeling. This forceful peeling caused both superficial and deep skin layers to slough off, stimulating an inflammatory reaction that caused redness and swelling of the face. Furthermore, harmful bacteria penetrated the skin, causing infection.
Chemical peels, also known as exfoliation, aim to remove dead skin cells, treat skin conditions, or address cosmetic skin problems such as acne, acne scars, enlarged pores, and hyperpigmentation. The specific chemical peel procedure depends on the skin condition and characteristics to select the appropriate active ingredient and concentration. Attempting to perform peels at home using the wrong ingredients and concentrations can lead to skin damage, peeling, blistering, and even serious infections. Without prompt medical attention, this can result in scarring and difficulty restoring the skin to its original state.
In the case of peeling for the purpose of exfoliation using low-concentration peeling products labeled as suitable for at-home peeling, people can do it at home. However, safety guidelines must be followed, such as: if peeling the face, lie with your head elevated 15-30 degrees, tie your hair back neatly, remove makeup and wash your face thoroughly, cover or close your eyes during peeling. When peeling, apply the active ingredient evenly, avoiding excessive repetition or applying too much to the eye sockets and corners of the nose...
"For purposes such as treating acne, scars, dark spots, enlarged pores... or if you don't know how to perform a peel at home, you need to see a doctor. If a doctor prescribes a peel, it will be performed by a healthcare professional with training in this technique to avoid unnecessary complications," Dr. Thao Hien advises.
