The accident happened on July 1st, causing Mr. T to be stunned, foreign objects flying into his neck and nose causing heavy bleeding that was difficult to stop. Here, the patient was given emergency treatment, temporary hemostasis bandage, pain relief IV, blood tests, and X-rays. The patient's vital signs at this time: Blood pressure dropped to 80/50 mmHg, pulse rate 110 beats/minute.
The doctors who examined him found that he was conscious, had pale skin and mucous membranes, his body was covered in blood, and he had slow contact. There were two main wounds, including a wound on the nose about 4cm in size, causing tears on both sides of the nostrils. The wound on the left side of the neck was 1.5cm in size and was still bleeding in a stream. When the wound was opened, there was a large swelling and blood clots under the wound.
X-ray results showed a fracture of the main nasal bone, a foreign body in the left neck area #10x13mm, suspected to be a bullet fragment. Consultation with a vascular specialist, an ENT doctor and anesthesiologist was conducted and the patient was transferred to the operating room for surgery.
Doctors diagnosed an open wound with a fracture of the main nasal bone and damage to the blood vessels in the left neck area with a foreign body causing hemorrhagic shock due to a suspected exploding bullet.
During surgery, the medical team explored and expanded the wound to remove all the blood clot under the wound in the left neck, found a ruptured superficial jugular vein and a torn internal jugular vein, and found a foreign object resembling a 10x12mm bullet shell.
The team cut and cleaned the vein wound, sutured and restored the vein. Hemostasis of the bleeding points, then closed the incision layer by layer, cut and cleaned and sutured the nose wound. Currently the patient is stable, blood pressure and pulse are within normal limits.
According to the assessment, this is a rare case of work accident. If first aid and pressure bandage are not applied promptly, the patient may lose a lot of blood and die.
