On the morning of December 2, the National Assembly continued to discuss in the hall the draft Resolution of the National Assembly on a number of breakthrough mechanisms and policies for the work of protecting, caring for and improving people's health; Investment policy for the National Target Program on health care, population and development for the period 2026 - 2035 (CTMTQG).

Commenting on this content, National Assembly delegate Tran Thi Nhi Ha   ( Hanoi City Delegation) believes that the regulations on regimes, policies, and salaries for medical staff in the draft has not really been a breakthrough in attracting and retaining qualified doctors. In reality, there is a serious shortage of doctors at the grassroots level, but the policies we are applying, such as sending young doctors to the grassroots level or seconding them for 2-3 years, are only temporary.

From there, delegates proposed to add solutions to attract doctors from the non-state sector to work at the grassroots health care level, needing to be considered for seniority and salary level equivalent to doctors working in the state sector. Doctors are considered for special recruitment into civil servants at health stations (without having to go through an exam) if they have a certificate or practice license.

Along with that, it is proposed to apply a 100% preferential allowance for doctors directly working at commune health stations and preventive medical facilities; for other medical professional positions, they are entitled to a minimum of 70%.

National Assembly delegate Nguyen Tam Hung (Ho Chi Minh City Delegation) proposed to supplement professional responsibility allowances according to each working position, not just according to the specialty, because at the grassroots health care level and in resuscitation and anti-poisoning units, work pressure, on-duty intensity and occupational risks are very high, but if responsibility allowances are not regulated, it will be difficult to ensure retention and create sustainable career motivation.

There should be no distinction in allowance levels with medical staff.

Participating in the discussion, National Assembly delegate Pham Khanh Phong Lan (Ho Chi Minh City Delegation) expressed "appreciation for the Party, State, Government and National Assembly always having policies to develop the health sector to better serve the people".

Commenting on the specific content of the salary policy, delegate Phong Lan said that the salary level 2 is just a formality, because level 1 and level 2 do not have a significant difference. "Each salary level represents 3 years of work without violations and only differs by about 0.3 - 0.4 times the basic salary coefficient. If it is given, then give it all, why not also raise level 3 and level 4?"

Delegates from Ho Chi Minh City also expressed concern that the subjects of application were limited to doctors, pharmacists and preventive physicians, while there were many other medical staff.

Regarding the preferential allowance, delegate Phong Lan said that there should be no distinction between 100% and 70% because in remote areas it is 100%, but the cost of living is much lower than in the expensive city. At the same time, the delegate also suggested that within the hospital, there should be no distinction between spending for any department such as resuscitation, emergency, pathology... but that the hospital should be autonomous and decide according to actual needs.

National Assembly delegate Tran Kim Yen (Ho Chi Minh City Delegation) proposed adding emergency staff outside the hospital to Clause 2, Article 3 so that emergency staff outside the hospital can also enjoy 100% preferential occupational allowance.

The delegate also proposed to remove point b, clause 3, article 3 so that those who regularly and directly work in medical profession at commune health stations and preventive medicine can enjoy 100% preferential occupational allowance, regardless of the level of 100% and 70%.

Proposal to support tuition fees for public medical doctor training

National Assembly Representative Tran Khanh Thu (Hung Yen Province Delegation) stated that in reality, to study medical doctor at public schools training in the health sector, the standard score has always been at the highest level in the past 10 years; the study time is long, the tuition fee is high because public schools operate financially autonomously, the main source of income is from tuition fees. Tuition fees at all schools are very high; beyond the ability of low-middle income families, this has become a barrier for many students who want to pursue a medical career.

Therefore, delegate Khanh Thu proposed to add the subject. Training medical doctors of public training institutions into the group of subjects guaranteed by the State budget, tuition support during the training period; with a commitment that after graduation, they will work according to the assignment of the State.

"This will ensure that students, especially those from disadvantaged backgrounds, have the opportunity to achieve their dream of becoming doctors, while also solving the problem of medical human resources in remote areas and areas lacking doctors," said delegate Khanh Thu.

Implement many regular solutions on regimes and policies for medical staff

Speaking to receive and clarify the opinions of National Assembly delegates, Minister of Health Dao Hong Lan said that when developing these two contents, the health sector considered many factors; closely coordinated with ministries, branches and localities to review the plans, improve feasibility to ensure both resources and implementation capacity.

Minister Dao Hong Lan said that regarding the delegates' opinions on the issue of periodic health check-ups and screening, the Ministry of Health has included it in the Law on Disease Prevention (expected to be passed at this session). Accordingly, in the process of guiding the implementation of the Law on Disease Prevention, from the subjects to the roadmap and specific implementation, the Government will guide.

Regarding the policy of free hospitalisation and diversified health insurance packages, the general principle is that the Government guides implementation. If the regulations are too detailed about the subjects and roadmap, when resources or implementation conditions change, the law or resolution cannot be amended immediately.

Regarding the opinion on the regime, salary policy and allowances for medical staff, according to Minister Dao Hong Lan, in addition to implementing Resolution 72-NQ/TW such as selecting the number of subjects and levels to gradually increase the allowance regime for medical staff, there are also regular tasks. Currently, the Ministry of Health is submitting to the Government to develop a Decree on special regimes such as: on-duty regime and other allowance regimes for medical staff in villages and hamlets...

"That means there are things that need to be included to guide the spirit of Resolution 72, the Ministry is still doing regular things" - Minister Dao Hong Lan said.
