Accordingly, late on August 5th, male patient D.VT (45 years old, residing in Mang Thít District, Vĩnh Long Province) was admitted to the hospital with severe abdominal pain in the right flank. According to records, the patient had been involved in a traffic accident two days prior, resulting in abrasions to the face and jaw accompanied by dull abdominal pain. The patient self-medicated but the pain did not subside and worsened, so he sought treatment at Vĩnh Long General Hospital.
Upon examination, the general surgery doctor found the patient's abdomen to be rigid like wood, with the most severe pain in the right flank. Therefore, they suspected a perforated hollow organ and ordered emergency laparoscopic surgery to locate the injury.
Upon endoscopy, doctors found the patient's appendix to be inflamed and congested, with a large amount of cloudy fluid and pseudomembranes.
Dr. Tran Van Nhieu, Head of the General Surgery Department, assessed that the abdominal condition was very serious and inconsistent with the signs of appendicitis, so he recommended transferring the patient to open surgery for a comprehensive assessment of the injury.
Subsequently, the surgical team dissected the right colon mesentery to expose the duodenum, where they found a large amount of bile and digestive fluids. After aspiration and thorough examination, they discovered a perforation in a segment of the duodenum approximately 3 cm in diameter. This was the cause of the patient's severe pain.
The doctors quickly sutured the perforation and inserted a duodenal drainage tube. After 4 hours of surgery, the patient was out of danger.
According to Dr. Nhieu, the duodenum is the first section of the small intestine, adjacent to the stomach, and plays a very important role in the digestive process, contributing to the absorption of nutrients by the body thanks to the bile and pancreatic enzymes that are secreted there.
Duodenal rupture is a particularly serious, rare, and difficult-to-diagnose emergency. A ruptured duodenum can obstruct the flow of food, causing food, as well as bile and pancreatic fluids, to spill into the abdominal cavity, leading to rapid infection and toxicity.
