A 33-year-old woman in Ho Chi Minh City was admitted to the emergency room with a swollen, shiny, purple-red buttock and severe pain, making it difficult to sit or walk.

Consequences after 7 years of filler injections to enhance buttock

On September 9th, Dr. Nguyen Phan Tu Dung, Director of Professional Services at JW Korean Hospital, stated that ultrasound and clinical examination results revealed numerous patchy hematomas of reddish-purple to dark purple color, some areas of skin suspected of lacking blood supply, with a risk of necrosis, and the entire affected area was edematous and lacked physiological elasticity.

When the doctor pressed on the buttocks for examination, the patient experienced intense pain, with abnormally hardened tissue, indicating a risk of deep abscess and potential widespread infection. MRI results revealed a massive abscess measuring 20 cm long and 15 cm thick, forming honeycomb-like cavities within the muscle tissue – a rare and particularly dangerous complication that threatens life if not treated promptly.

Upon taking her medical history, Ms. T. stated that 7 years ago she had received "collagen filler" injections to enhance her buttocks. Two months ago, her buttocks began to show signs of pain, swelling, and unusual redness. Her family took her to an imaging diagnostic center for examination and was advised to seek treatment at a specialized facility, so she came to JW Hospital for treatment.

Dr. Nguyen Phan Tu Dung determined that this was a serious case, the abscess had eaten deep into the muscle tissue, forming a honeycomb structure, with the risk of spreading and causing life-threatening sepsis, so he requested the team to perform emergency surgery that same day.

"In this critical condition, we were forced to proceed with an emergency treatment protocol. The entire abscess will be thoroughly drained, continuously irrigated, and all filler and necrotic tissue removed. The patient will continue to receive high-dose antibiotics combined with the placement of a VAC negative pressure aspirator to drain the pus. This process needs to be maintained for at least 7 days, while closely monitoring for the risk of infection and spreading necrosis , " Dr. Tu Dung further shared.

As soon as the surgery began, the entire team was shocked because with just a 2cm incision, pus continuously gushed out from the patient's buttocks. The team had to continuously use a high-intensity suction system to clean up the huge amount of pus inside the surgical field. At the end of the surgery, nearly 2,500 ml of pus of unknown origin mixed with necrotic tissue was collected.

Dredged out dozens of discolored filler particles

The second case involves a female patient who received "multi-layered filler" injections at a spa two years ago. The patient reported that just over a year after the injections, her buttocks became stiff and painful to the point where she couldn't walk or even sit comfortably. She then went to a cosmetic clinic to have the silicone removed and buttock implants placed. However, after a short time, her buttocks became painful and swollen again. Worried, she then went to JW Hospital for examination and treatment.

After performing diagnostic imaging tests, Dr. Tu Dung determined that the filler had not been completely removed, with residual material still remaining inside the patient's buttocks, appearing as scattered lumps throughout. The biggest challenge in this case is that if the foreign substance is completely removed carelessly, it could cause a perforation of the buttocks and affect aesthetics. However, if it is not completely removed, the patient cannot be treated definitively.

After 3 hours of surgery, the JW Hospital team removed dozens of discolored, sticky, and bloody filler particles from the patient's buttocks. These were the cause of inflammation, pus accumulation, and tissue necrosis, causing the patient severe pain and the risk of losing her buttocks.

Dr. Nguyen Phan Tu Dung advises people to absolutely avoid filler injections at unlicensed spas or beauty clinics. Low-quality fillers can cause infections, abscesses, necrosis, and severe complications. All beauty treatments should be performed at specialized hospitals with properly trained doctors and medical- grade materials. In particular, before undergoing any service, you must verify the information yourself. Absolutely do not blindly trust advertisements online.
