Strengthening human resources and improving state management of health at the grassroots level

Implementing the two-level local government model, Ha Tinh has quickly arranged and stabilized the organizational structure for communes and wards after the merger, thereby effectively implementing state management in all fields. However, due to many specific factors, state management in the health sector at communes and wards still faces many difficulties due to the lack of specialized human resources. In this situation, the Ha Tinh Department of Health issued Decision No. 1366/QD-SYT dated November 28, 2025 on the secondment of civil servants to work at the People's Committee at the commune level.

Duc Tho Medical Center met and encouraged officials assigned to work in communes.

According to the decision, 64 officials currently working at units under the Department of Health will be assigned to 64 communes and wards in the province for a period of 1 year, starting from December 1, 2025. The officials assigned are mostly experienced officers in functional departments and specialized centers such as preventive medicine, population, food safety, health inspection, etc.

This is an important step in preparing human resources for the commune level in the context of many new tasks of state management in the health sector being decentralized to local authorities. In particular, according to Circular 43/2025/TT-BYT, commune health stations become public service units under the People's Committees of communes, requiring localities to have a team of staff with good medical expertise to advise and organize effective implementation. The secondment of health officials is considered "timely" when localities are facing many new requirements on management and organization of health tasks.

Doctor Nguyen Dinh Thien - Director of Duc Tho Medical Center said: In this secondment, the unit has 5 officials assigned to communes, including 4 officials from the center's departments and 1 official from the health station. These are officials with good professional capacity and experience, so when they are seconded to the commune-level People's Committees, they will provide good support for advisory work in state management in the health sector in accordance with the operation of the 2-level local government model.

The dispatch of health officials to the commune level will contribute to improving the effectiveness of primary health care consultation for the people.

According to the decision, the People's Committee at the commune level has seconded officials responsible for receiving and assigning work appropriate to the qualifications and capacity of each official; at the same time, creating conditions for this team to effectively promote their expertise and advisory skills, contributing to improving the quality of medical activities at the grassroots level.

Mr. Ho Quoc Tuan - Head of the Department of Culture and Society, Thanh Sen Ward said: Being a central area with a large population and large private medical and pharmaceutical activities, there is an urgent need for state management in the health sector. Therefore, when an officer is assigned to the ward, it will contribute to improving the effectiveness of advisory work and reducing the pressure on health management in the area.

Agreeing with the above opinion, many communes and wards also said that, in recent times, the workload in the health sector at the commune level has been very large, from preventive medicine, food safety to population, expanded immunization. The increase of specialized staff from the Department of Health is a very practical support, helping localities reduce pressure and perform tasks more methodically and in accordance with regulations.

Population and family planning tasks at the commune level will be promptly consulted when health officials are seconded.

It is known that immediately after receiving the project, the People's Committee of the commune/ward assigned seconded officials to be in charge of appropriate fields, participating in developing the 2026 health plan, and at the same time supporting the health station in implementing professional programs.

Mr. Le Chanh Thanh - Deputy Director of the Department of Health said: "The dispatch of 64 officials to work at the People's Committees at the commune level is not only to supplement human resources but also to help localities quickly access and grasp new regulations on the health sector's organizational model. This is a necessary solution for the grassroots health system to operate smoothly, especially in the period of transferring a number of tasks from the old district-level government to the commune-level government. The dispatched officials will be an important bridge between the health sector and the commune-level government, helping state management work in the health sector to be deployed in a more unified, synchronous and effective manner. This is also an opportunity for officials to access grassroots practices, improve their capacity, professional and management skills".

It is known that, based on the Government 's guidance, the Department of Health is currently advising the Provincial People's Committee to establish 69 commune-level health stations on the basis of rearranging 209 existing health stations and transferring them to the Commune-level People's Committee for management. At the same time, additional functions, tasks and human resources from the preventive medicine and population sectors of current health centers are added. This secondment of officials is considered an important preparation step, creating stable operation and unifying state management of health, contributing to strengthening the grassroots health care foundation in people's health care.
