Spectacularly saving many difficult stroke cases

Deputy Minister Tran Van Thuan said that after only 5 years of operation, the Stroke Center has become a leading trusted address, recognized by the World Stroke Organization as meeting international standards. But what is more valuable is that tens of thousands of patients have been saved in time, rehabilitated early, and given more opportunities to live a full life.

"The silent efforts of each shift, each red alert chain, each emergency treatment process are the silent bricks that build the Vietnamese medical brand in the new era" - Deputy Minister Tran Van Thuan emphasized.

Not long ago, a 103-year-old patient with a history of hypertension and a cerebral infarction due to left middle cerebral artery occlusion was transferred to the Stroke Center within a 1.5-hour window by his family and a lower-level medical facility with symptoms of right hemiplegia and impaired consciousness. The patient's NIHSS score (a score that shows the patient's neurological deficit, the higher the score, the more severe the patient, and the higher the risk of sequelae and death) was 20.

Cerebral angiography showed acute left hemisphere cerebral infarction due to occlusion of the middle cerebral artery segment M1, with ASPECT score of 8/10. Immediately, an emergency consultation was performed and a decision was made to treat with thrombolysis at a dose of 0.6 mg/kg and mechanical thrombectomy after discussion and agreement with the family.

Deputy Minister of Health Tran Van Thuan, authorized by the Prime Minister, awarded the Prime Minister's Certificate of Merit to the Stroke Center - Bach Mai Hospital.

Experts at the Stroke Center said that even in developed countries around the world, 103-year-old patients are at very high risk when performing 2 reperfusion treatments because of concerns about the risk of bleeding and complications.

However, with great effort and expertise, after only 45 minutes of intervention, Bach Mai doctors were able to achieve the highest and best recanalization. The patient improved very well and returned to almost normal clinical condition before and was discharged after 7 days of treatment.

And these are also two techniques that Bach Mai Hospital has regularly trained for doctors and medical staff for grassroots hospitals, contributing to the professional development of the stroke emergency system at provincial and regional hospitals. At the same time, it also contributed greatly to the success in registering and receiving the certification of "Specialized Stroke Center" of the World Stroke Association.

The Stroke Center is also the place that successfully saved the first clinical case in Vietnam using dabigatran (a new generation anticoagulant) with acute cerebral infarction that was treated with thrombolysis, thanks to the application of a specific neutralization treatment method; A case of critical cerebral hemorrhage recovered spectacularly thanks to the hypothermia technique in the treatment of cerebral stroke;

At the same time, the Center's doctors have mastered the technique of emergency external ventricular drainage - a specialized stroke technique that helps save the lives of many patients with intraventricular hemorrhage...

Director of Stroke Center (Bach Mai Hospital) Mai Duy Ton discussed with Professor Jeyaraj Durai Pandian - President of World Stroke Association,

Affirming Bach Mai's position on the international stroke map

According to Associate Professor, Dr. Dao Xuan Co - Director of Bach Mai Hospital, one of the important milestones affirming the leading position of the Stroke Center - Bach Mai Hospital in the field of stroke in Vietnam and the region is that the Center is honored to become the first unit in Vietnam to be awarded the "Advanced Stroke Center" certificate by the World Stroke Organization (WSO) - a rigorous certification based on compliance with global standards based on evidence, process quality, professional competence and patient treatment results.

This is the highest certification in the WSO assessment system, requiring strict criteria on facilities, human resources, diagnosis and treatment processes, training activities, scientific research, as well as development of emergency and stroke treatment networks.

At the same time, the first Global Stroke Alliance (GSA) 2025 Conference held in Asia took place in Hanoi as a recognition of Vietnam's achievements from a country with many difficulties in stroke prevention in previous years to quickly becoming a country with a high position in the regional and world stroke system, also an international recognition of creativity and innovation in stroke care, creating a strong influence for stroke prevention and control activities in Southeast Asia.

The Party Committee and Board of Directors of Bach Mai Hospital congratulate the Stroke Center on its 5th anniversary.

At GSA 2025, the World Stroke Association awarded the "Specialized Stroke Center" certificate to Bach Mai Hospital. This is the first certificate awarded by the World Stroke Association to a hospital in Vietnam, creating a turning point in standardizing the quality of stroke treatment in Vietnam.

"These results have affirmed Bach Mai's position on the regional and international stroke map" - Associate Professor, Dr. Dao Xuan Co proudly said.

In the period of 2024-2025, Bach Mai Hospital Stroke Center has successfully implemented the Project "Enhancing medical care and rehabilitation training". This is a large-scale, strategic project, focusing on improving the capacity of the health system, training human resources and developing a comprehensive care and rehabilitation model for stroke patients in Vietnam.

Up to now, each year, the Center receives about 12,000 - 13,000 stroke patients, mainly severe and complicated cases transferred from satellite facilities, in which, closely coordinating with clinical units in Bach Mai Hospital to fully implement specialized techniques in emergency resuscitation and treatment of stroke patients, including: cerebral reperfusion treatment with thrombolytic drugs; applying specialized stroke patient resuscitation techniques (targeted body temperature control in cases of stroke patients with uncontrolled hyperthermia; external ventricular drainage, with or without ventricular fibrinolysis; transcranial Doppler ultrasound; invasive and non-invasive intracranial pressure monitoring...).

According to Associate Professor, Dr. Dao Xuan Co, today's achievement is the crystallization of expertise, discipline, compassion and innovation of the Bach Mai Stroke Team - a multi-specialty team.

Entering a new stage of development, the Director of Bach Mai Hospital requested the Stroke Center to continue to strengthen quality according to WSO standards; expand the pre-hospital stroke emergency network, increase tele-stroke; At the same time, promote clinical research and big data, apply AI in prediction and individualize treatment, thereby contributing to reducing mortality, disability and social burden due to stroke.
