Family and Society - The child was brought to the hospital with a nearly complete detachment of the right earlobe, a torn outer ear canal cartilage and ear cartilage, along with many other injuries on the body due to a dog bite.

On February 4th, information from Hanoi Children's Hospital indicated that doctors at the hospital had successfully received and treated a child whose earlobe was nearly severed due to a dog bite.

Accordingly, the patient is an 8-year-old boy (from Hanoi) who was brought to the hospital by his family in a critical condition. Multiple soft tissue injuries to the right ear, scalp, right forearm, and hand, along with numerous abrasions caused by the dog bite.

Upon examination, doctors found a near-complete detachment of the right earlobe, a torn outer ear canal cartilage and auricular cartilage, with a 2.5 cm skin bridge remaining at the earlobe; multiple deep bite marks and numerous deep lacerations extending to the subcutaneous layer on the head and right arm, the longest laceration being approximately 5 cm.

According to the family, the child was playing at her grandmother's house and was bitten by the family's pet dog. Immediately after the accident, the child was taken to Chuong My Hospital for first aid, wound dressing, and temporary bleeding control. Coordination was then carried out, and the child was immediately transferred to Hanoi Children's Hospital.

According to Dr. Hung Anh, from the General Surgery Department of Hanoi Children's Hospital, upon receiving the patient, the medical team quickly completed the tests and performed emergency surgery on the child.

The surgery aims at two main goals: maximizing cleanliness and removing damaged tissue to prevent infection, and preserving the auricle as much as possible, ensuring the ear's shape and the structure of the ear canal.

During the surgery, the doctors found that the child's earlobe was severely damaged, with extensive soft tissue damage, complete rupture of the cartilage and blood vessel structures from the ear canal to below the earlobe, and insufficient blood drainage from the skin bridge at the earlobe, as evidenced by the cyanotic and heavily bleeding skin flap on the periphery.

The child underwent surgery to remove damaged tissue, suture the external auditory canal cartilage and auricular cartilage, and reconnect the ear veins using microsurgical techniques. Post-surgery, the ear's shape was satisfactory, the auricle was warm and pink, with a normal color and no cyanosis. Other wound sites were repeatedly irrigated, debrided, and sutured to remove excess skin.

The child is now fully alert, and their general condition and surgical wound are stable. The child is receiving antibiotics, wound dressing changes, and rabies and tetanus vaccinations.

According to doctors at Hanoi Children's Hospital, for this child, they performed microsurgery to treat the nearly severed earlobe. This is a highly complex surgery, requiring sophisticated and specialized equipment and instruments such as surgical microscopes, microsurgical instruments, and microsurgical sutures to connect and graft small structures like blood vessels and nerves.

This method is a solution for many complex injuries such as limb amputation, extensive injuries requiring reconstruction with microsurgical flaps, vascular and nerve injuries in trauma, etc. It is a powerful tool that gives surgeons many options and the best treatment plan for patients.

Warning: Dangerous accidents can occur when bitten by a dog.

According to doctors, accidents caused by animal bites are common, especially among children, due to their curiosity and their inability to recognize the dangerous signs of animals.

Animal bites can have serious consequences such as: damage to blood vessels and nerves, infection of the bite wound, and especially serious cases like rabies or amputation of body parts such as: genitals, ears, nose, limbs… In addition, it has a significant impact on the child's psychology, therefore family members need to have plans to prevent accidents for young children.

Therefore, when a child is bitten or licked by an animal, parents or caregivers should take note of the following:

- Wash the wound with clean water and soap for 15 minutes.

- Disinfect the wound with alcohol or betadine.

- Apply a bandage gently to stop the bleeding.

- Take your child to a healthcare facility for check-ups and vaccinations.

- Monitor the animal that bit the victim for 15 days.

Note: Do not attempt to stitch the wound yourself to avoid worsening the child's condition.
