Successfully maintained 18-week pregnancy thanks to uterine cerclage technique

(DNO) - Doctors of the Department of Obstetrics and Gynecology - Da Nang Family General Hospital (FAMILY) have just successfully performed an emergency case of uterine cerclage, preserving an 18-week-old fetus in a serious state of threatened miscarriage.

Ms. TN (33 years old) was 18 weeks pregnant and was taken to the Family General Hospital for emergency care by her family with symptoms of mild abdominal pain.

The examining doctor noted that the pregnant woman's cervix had dilated to 4cm, the amniotic sac was prolapsed, and she was at risk of miscarriage. This is an extremely dangerous obstetrical event, often leading to late miscarriage or extremely early premature birth when the fetus is not yet able to survive outside the uterus. Immediately, the doctors of the Obstetrics and Gynecology Department urgently consulted to handle the situation.

MSc. Dr. Nguyen Thi Thanh Loan - Head of Obstetrics and Gynecology Department - Family General Hospital said that when the fetus was admitted to the hospital, it was only 18 weeks old (more than 4 months). If it had to be born at this stage, the baby's chance of survival would be extremely slim. This situation requires a quick decision and specialized intervention techniques to keep the fetus safe in the mother's womb.

After an emergency consultation, the medical team decided to perform an emergency cervical cerclage. This is a complex procedure indicated in cases where the cervix is ​​dilated and there is a very high risk of premature birth, with the aim of holding the cervix tight and preventing the fetus from falling out.

Before the procedure, the mother is treated with uterine contraction-lowering drugs to stabilize contractions and minimize the risk of amniotic rupture during the intervention.

In the operating room, with great concentration, the doctors skillfully pushed the prolapsed amniotic membrane back into the uterine cavity and placed a sturdy ring of suture around the cervix, like "sealing" the door that was opening early for the mother.

According to Dr. Nguyen Thi Thanh Loan, emergency cervical cerclage is a big challenge because it requires high skills and careful preparation, especially when the cervix is ​​already dilated and the fetus is still very premature. "This success is not only a joy for us and the pregnant woman's family, but also a hope for many pregnant women facing the risk of premature birth" - Dr. Nguyen Thi Thanh Loan shared.

After the procedure, the pregnant woman's condition gradually stabilizes, uterine contractions are controlled, and the fetus continues to develop safely in the womb.

Currently, the patient continues to be monitored. The doctor also continues to use support methods and advises on appropriate rest and nutrition to ensure the pregnancy is maintained until full term. The stitches will be removed around week 36-37 of pregnancy or when there are signs of real labor, so that the baby can be born safely.

Most miscarriages due to cervical incompetence are asymptomatic and are discovered incidentally during prenatal check-ups. Therefore, regular prenatal check-ups, especially in the second trimester, are very important for prevention.

Emergency cervical cerclage is a difficult procedure and should be performed at large obstetric facilities, with close coordination between obstetricians and anesthesiologists to ensure the safest and best treatment for both mother and fetus.
