The first case involves a patient who had received a cheap, multi-layered collagen filler injection to her buttocks at a local spa two years ago. Just over a year later, her buttocks became so stiff and painful that she couldn't walk or even sit comfortably.
The patient had previously undergone silicone removal and buttock augmentation at a cosmetic clinic, but the symptoms kept recurring. Upon arriving at JW Korean Hospital (Ho Chi Minh City), MRI and ultrasound results revealed a significant amount of foreign matter in the form of clumps scattered throughout the muscle tissue in the buttocks.
The medical team performed a three-hour surgery, removing dozens of discolored, thick, pearl-like filler particles mixed with blood and pus. This was the cause of the infection, pus accumulation, and risk of tissue necrosis. After the surgery, the patient was placed under negative pressure suction and will undergo a second surgery a few days later.
The second case involves Ms. T. (33 years old, residing in Ho Chi Minh City) whose left buttock was swollen, taut, reddish-purple, and so painful that she was unable to walk.
Examination and ultrasound results revealed multiple patchy bruises, areas of ischemic skin, and a high risk of necrosis. Upon palpation, the tissue appeared abnormally hard, indicating a risk of deep abscess formation and widespread infection.
MRI revealed that Ms. T. had a massive abscess measuring 20cm long and 15cm thick, penetrating deep into the muscle tissue and pelvic bone, forming a "honeycomb" structure. This is a rare but extremely dangerous complication.
The patient stated that she had received "collagen filler" injections to enhance her buttocks 7 years ago. For the past two months, her buttocks have become painful, swollen, and red. Her family advised surgery, but she was worried about unsightly scarring, so she sought out JW for cosmetic treatment.
Dr. Nguyen Phan Tu Dung, Medical Director of JW Hospital, said: "The abscess had penetrated deeply and posed a risk of sepsis, so we had to perform emergency surgery. All filler and necrotic tissue were removed, continuously irrigated, and then a VAC negative pressure suction device was used to drain the pus."
During the surgery, despite only a 2cm incision, pus continuously gushed out. The surgical team had to use a high-powered suction machine to remove nearly 2.5 liters of pus mixed with necrotic tissue.
These two consecutive cases of complications serve as a warning to anyone who still believes in advertisements promising "quick and cheap beauty treatments." Just one questionable injection can cost you your health, or even your life.
Never get filler or dermal filler injections at unlicensed facilities.
According to Dr. Tu Dung, "cheap fillers, multi-layered collagen, and fillers of unknown origin" are often widely advertised online and injected at unlicensed spas and beauty clinics. "These substances can cause inflammation, abscesses, necrosis, granuloma formation, leaving severe sequelae, and even threatening life," he emphasized.
Doctors strongly advise people against getting filler injections at unlicensed facilities. If you need cosmetic procedures, you should go to a specialized hospital, have the procedure performed by a licensed doctor, and use medical- grade materials.
