However, this disease is getting younger and younger, appearing in school-age children due to the pressure of studying and taking exams, especially at the end of the school year.

Bach Mai Hospital has received and treated many cases of students suffering from this disease, especially cases with serious complications.

A typical case of peritonitis due to perforated duodenal ulcer was treated surgically at the Department of Gastrointestinal - Hepatobiliary - Pancreatic Surgery.

The male patient is 15 years old, a 9th grade student preparing for the final exam to enter 10th grade.

I have a history of gastritis and duodenitis and have been treated with internal medicine many times. Recently, I have been worried about exams and studying for exams, so I have more pain in the upper abdomen.

After an evening class, NXĐ returned home with a severe stomachache and high fever.

The family took D to Bach Mai Hospital for emergency treatment in a state of obvious infection, with a painful, stiff abdomen.

After the necessary tests, patient D was diagnosed with peritonitis due to perforated duodenal ulcer.

Immediately afterwards, patient D underwent emergency laparoscopic surgery to suture the perforated duodenal ulcer, clean and drain the abdomen.

Patient D was discharged after 5 days of treatment but still had to continue medication and follow-up visits.

Perforated ulcer is a serious complication of peptic ulcer disease and is the result of an imbalance between the attacking factors (acid, pepsin) and the protective system of the gastric and duodenal mucosa.

Every year, about 4 million people in the world suffer from it, accounting for 1.5-3% of the world's population. The complication rate of gastric and duodenal ulcers is about 10-20%, of which perforation complications account for 2-14%.

Perforated duodenal ulcers can occur at any time of the year but are more common in winter and spring, the transitional season. Perforated duodenal ulcers detected and treated late have a mortality rate of 2.5-10%. In elderly patients, the mortality rate is up to 30%.

Identifying stomach and duodenal pain

For school-age children, parents and relatives need to recognize early signs of stress, anxiety, and even fear in children such as: fatigue, nervousness, anxiety, sweating, discomfort, restlessness, emotional disturbances (irritability, frustration, overreaction to normal events), poor sleep and appetite, or abdominal pain and diarrhea when stressed...

When having gastric ulcer, children will have dull abdominal pain above or around the navel, similar to digestive disorders, so parents often subjectively treat it themselves with digestive enzymes, deworming... so many times the disease is only discovered when it has complications. In addition, children may also have nausea, vomiting, belching, heartburn...

To help children avoid pressure and stress, parents need to help their children have a reasonable study plan, avoid concentrating too much before exams, have time to rest, relax, and exercise. Eat scientifically , hygienically, live a regular life, and avoid staying up late.

Encourage and motivate children to create a comfortable mood, do not demand results that are far beyond their actual abilities. Do not scold or insult children when the results do not meet expectations.
