Medical news on July 12: Blindness and necrosis due to cosmetic filler injection

After injecting filler to fill in a sunken forehead and lift her breasts, two women had to be taken to the emergency room at Viet Duc Friendship Hospital. One was blind, the other had an infection and necrosis in both breasts.

Serious complications due to missing the golden time

Associate Professor, Dr. Nguyen Hong Ha, Head of the Department of Plastic Surgery, Viet Duc Friendship Hospital, said that doctors have just treated two cases of serious complications due to cosmetic filler injections, one of which was injected abroad.

Doctors of Viet Duc Friendship Hospital are examining and treating people.

Ms. D.TN (30 years old, Ha Tinh ) is currently living and working in Japan. A few days ago, to have a beautiful forehead, she injected filler into her forehead and temples at a spa in Japan. When she just injected 0.5cc into the middle of her forehead, she felt droopy eyelids, dizziness, and vomiting.

She was immediately injected with an antidote but still felt uncomfortable and nauseous. She went to the hospital for a check-up, was examined by a doctor and did not intervene, and was sent home for observation.

The next day, she saw her eyes were swollen and red, so she went to the hospital for emergency treatment. At that time, her eyes were no longer clear. Realizing that her condition was getting worse very quickly, almost completely blind, she decided to book a ticket back to Vietnam.

The female patient came to the Central Eye Hospital for examination, then transferred to Viet Duc Hospital. According to Associate Professor Ha, this was a very serious accident and the patient came to the hospital late (6 days after filler injection), causing complications to the eye's vascular system, leading to loss of vision in the right eye, with signs of muscle necrosis and tissue necrosis around the eyeball.

Because the patient had missed the “golden time”, it was extremely difficult to restore her vision. Doctors intervened with the most modern methods to help the girl find the light.

After 6 days of vascular intervention, the female patient's right eye has slightly improved and can now move the eyeball gently as requested by the doctor. However, the patient still needs long-term treatment and whether or not her vision can be restored is still difficult.

The second case that had to be hospitalized at the same time was Ms. NCT (31 years old, Quang Nam ). She was hospitalized with abscesses on both sides of her breasts due to filler injections for breast augmentation.

Previously, through social networks, Ms. T. saw a beauty salon advertising a quick and effective breast augmentation filler injection method, so she went there. After the injection, she saw lumps in her chest and often felt pain and swelling.

After 3 years of filler injections, she went to the hospital for a cancer screening package. Here, the doctors said that her breasts were not clear if they had cancer or fibroids, but had filler tumors.

Ms. T. wanted to have another baby, but because she could not determine what substance had been injected into her breasts before, the doctor advised her to have surgery to remove the fillers, but it had to be done at a central hospital.

However, Ms. T. went to a beauty salon to have fillers removed. After the filler removal procedure, Ms. T. experienced pain and high fever, and antibiotics did not help. At this time, because of the pain and fear, she went to Viet Duc Hospital.

Associate Professor, Dr. Nguyen Hong Ha said that Ms. T. came to the hospital in a state of infection and poisoning, and was diagnosed with a breast abscess with filler tumors throughout both mammary glands, possibly caused by bacteria entering through filler injections and suction.

Ms. T.'s symptoms of chills and fever signaled that the abscesses were about to burst and if they entered her lungs, it would be life-threatening. Doctors quickly performed surgery to remove the filler from her body.

Through this, doctors warn that there are many fake fillers and smuggled fillers on the market. Women who want to beautify themselves do not go to licensed medical facilities, but choose spas and beauty salons that operate illegal filler injections to perform procedures.

When the person injecting filler is not a specialist, has no knowledge of cosmetic surgery, and is only a spa employee injecting filler, the risk of injecting into the blood vessels around the eye socket is very high.

At that time, the patient is at risk of blindness, or cerebral embolism causing stroke, endangering life. In addition, because the facilities perform filler injections illegally, the tools are not sterilized according to regulations, leading to bacteria entering and causing infection, abscess, necrosis, and even death because there have been cases of infection with drug-resistant bacteria.

Therefore, when doing beauty treatments, women must go to reputable, licensed medical facilities with specialized doctors to ensure safety.

Consecutive cases of serious cosmetic accidents

In just a short period of time, in Ho Chi Minh City, there have been many serious complications after cosmetic surgery, most of which are liposuction.

For example, according to the Ho Chi Minh City Department of Health, the Diva Saigon Cosmetic Clinic has committed a series of violations. And the Department has issued a document announcing the suspension of the Diva Saigon Cosmetic Clinic's operations and transferred the case file to the Ho Chi Minh City Police for investigation.

Or another facility is the Paris Cosmetic Dentistry Hospital (Ba Huyen Thanh Quan Street, District 3) for abdominal liposuction and excess skin removal surgery. At 1am on July 5, the patient had difficulty breathing, chest pain, dry cough... and was transferred to Cho Ray Hospital with respiratory failure, acute pulmonary alveolar-capillary damage (ARDS) suspected of fat embolism.

Diagnostic imaging results showed that the patient had dilated pulmonary arteries, pulmonary hypertension, alveolar damage, diffuse consolidation of both lungs with inflammation, small bilateral pleural effusion, and passive collapse of adjacent lung parenchyma. The patient was transferred to the Intensive Care Unit (ICU) for treatment.

Patient THA (born in 1999, in Can Tho) had liposuction at The Beauty Clinic (District 1) on July 5. After the procedure, the patient felt tired, dizzy, had a headache, and had severe abdominal pain. The patient went to An Binh Hospital but his condition worsened so he was transferred to Cho Ray Hospital at 5:49 a.m. on July 6.

Upon admission, the patient was conscious, with pale skin and mucous membranes, a purple swelling of the left abdominal wall radiating to the back; three 1 cm abdominal incisions with bleeding that soaked the gauze, and one incision on the back. The patient was diagnosed with hypovolemic shock - abdominal wall hematoma, postoperative day 1 of liposuction.

Diagnostic imaging results showed hematoma in the subcutaneous tissue of the left abdominal wall, with signs of vascular leakage with unclear arterial branches inside the fluid collection, air in the subcutaneous tissue of the left abdominal wall; Fluid collection, fat infiltration of the subcutaneous tissue of the right abdominal wall.

Previously, on the morning of June 29, female patient VTH (born in 1977, in Khanh Hoa) came to Kangnam Saigon Cosmetic Hospital for examination with the intention of reducing fat and excess skin in the abdominal area.

At 12:00 noon on the same day, the patient was anesthetized by the TTNP doctor and had a total abdominoplasty combined with liposuction performed by the HQH doctor. After more than 4 hours of surgery, at 5:00 pm on June 29, the patient was extubated and transferred to the Department of Plastic Surgery for further treatment.

More than 1 day after surgery, the patient complained of fatigue, difficulty breathing, and his condition gradually worsened. The patient was transferred to Cho Ray Hospital. After emergency intervention at Cho Ray Hospital, the patient was temporarily stable again.

It is known that every year in our country, 250,000 people have cosmetic surgery, of which about 25,000-35,000 cases have complications, accounting for 14%.

Therefore, to ensure safety, the authorities recommend that when choosing a beauty facility, people should choose a specialized plastic surgery clinic or a reputable hospital, with experienced doctors, full practice certificates and operating licenses, modern equipment, and operating rooms with strict infection control procedures, which will minimize the risk of infection during cosmetic surgery...

The Ho Chi Minh City Department of Health recommends that when people discover or suspect an unlicensed medical examination and treatment facility or information advertising medical services on mass media, social networks, etc., they can immediately call the hotline at 0989.401.155 or report via the "Online Health" app so that the Department of Health Inspectorate can have information and promptly detect and handle it.
