Diabetic coma but not following treatment

Duc Giang General Hospital ( Hanoi ) recently admitted a child patient to the emergency room in a coma due to very high blood sugar. The patient is NTTG (12 years old, in Gia Lam District, Hanoi), with a history of type 1 diabetes but did not comply with treatment.

Two days before being admitted to the hospital, the patient showed signs of fatigue, thirst, vomiting, and abdominal pain, but the family did not treat him. When the child appeared to be very tired and lethargic, the family took him to the Pediatric Intensive Care Unit, Duc Giang General Hospital, in a coma, rapid breathing, and dehydration.

Doctors at Duc Giang General Hospital treated a 12-year-old patient with ketoacidosis due to type 1 diabetes.

At the hospital, through examination and testing, doctors found that this was a case of ketoacidosis coma in a diabetic patient who did not comply with treatment, with a high blood sugar level of 28 mmol/l, and blood gas showing severe metabolic acidosis.

The patient was promptly treated with active measures: intravenous fluid infusion, maintenance of intravenous insulin, and correction of acid-base and electrolyte imbalances. After 3 days of treatment, the patient's condition gradually improved, he was more alert, able to eat and drink, and his blood sugar was controlled with subcutaneous insulin injections with a regimen of 4 injections per day.

At the end of 2023, doctors at Duc Giang General Hospital also treated a case of diabetic ketoacidosis coma in a 4-year-old child living in Long Bien District. About 3 weeks before being admitted to the hospital, the child lost 3 kg and urinated frequently at night, then appeared increasingly tired, had rapid breathing, and exerted himself a lot.

Test results at the hospital showed that the patient was in a state of severe acidosis, with a very high blood sugar level of 37 mmol/l. Diabetic ketoacidosis is a serious complication of diabetes that occurs when the body produces too much acid in the blood called ketones.

According to Duc Giang General Hospital, type 1 diabetes in children is an endocrine disease that is rare in children. Failure to comply with treatment will cause serious complications that can be life-threatening if not treated promptly. Therefore, parents with children with diabetes need to have their children comply with treatment and have regular check-ups as scheduled to avoid serious complications and ensure the best quality of life for their children.

At the same time, when detecting signs of eating a lot, drinking a lot, and losing weight in children, it is necessary to see a doctor early to promptly detect diabetes in children.
