Doctors at Thu Duc General Hospital have just saved the life of a 78-year-old female patient, weighing 100 kg, residing in An Khanh ward (HCMC), suffering from many complicated underlying diseases, suffering from cardiac arrest and respiratory arrest thanks to multidisciplinary intensive resuscitation and the application of modern blood filtration techniques.

Previously, at the end of October, Ms. NTT was hospitalized with a high fever, back pain, difficulty breathing, and low blood pressure. Her medical history showed that she had a series of complex underlying diseases: heart failure, high blood pressure, type 2 diabetes, sleep apnea syndrome, and severe obesity - factors that significantly increase the risk of severe progression and death when infected.

At the Emergency Department, the patient was admitted in a lethargic state, with profound hypotension, requiring a combination of noradrenaline and adrenaline in high doses. Doctors diagnosed Ms. T. with septic shock from the urinary tract, acute respiratory failure, acute adrenal insufficiency and multiple organ failure, and then quickly transferred her to the Cardiovascular Intensive Care Unit for intensive intervention.

Despite intensive resuscitation, the patient's condition continued to worsen and he quickly fell into cardiac arrest and respiratory arrest. The on-call team immediately performed chest compressions, intubated him, and provided ventilator support. After nearly 15 minutes of emergency care, the patient's heart rate and spontaneous circulation were restored, bringing him back from the fragile border between life and death.

Ms. T. was in a "dead end" situation many times due to many underlying diseases and severe obesity.

Abdominal CT scan results showed hydronephrosis, pus in the right kidney, suspected ureteral obstruction - identified as the main source of infection. The Department of Urology was quickly invited for consultation and decided to place an emergency JJ ureteral stent to relieve pressure, drain urine and pus, reduce pressure and control the source of infection.

However, on the background of severe infection and many chronic diseases, the patient developed acute renal failure, severe metabolic acidosis, and still had to maintain two high-dose vasopressors, with the risk of life-threatening multiple organ failure. In this situation, doctors prescribed continuous hemodialysis (CRRT - Continuous Renal Replacement Therapy) with Oxiris adsorption filter, which both replaced kidney function and supported the removal of toxins and inflammatory mediators.

The patient was continuously dialysis for many days, combined with mechanical ventilation, appropriate antibiotics, correction of electrolyte disorders, blood sugar control, cardiovascular function support and 24/7 care and close monitoring.

After about 5 days, Mrs. T's blood pressure gradually stabilized, decreased, and then stopped vasopressor medication. Her kidney function improved, and her urine output increased, allowing her to stop dialysis after 10 days. On the 14th day, the patient was taken off the ventilator and extubated.

After 20 days of intensive treatment, she was discharged from the hospital in a conscious state, with stable vital signs, able to sit up and move around in bed.

According to Dr. Vu Quynh Tran - Cardiovascular Intensive Care Department (directly treating patient T.), this is an "extremely critical case, with a very low chance of survival" because the patient has just had cardiac arrest, multiple organ failure, many underlying diseases and is severely obese, making the process of resuscitation, care, weaning off the ventilator and removing the endotracheal tube very difficult.

"Thanks to close coordination between many specialties and the application of modern resuscitation and blood filtration techniques, the patient overcame the critical stage and recovered well" - Dr. Tran shared.

According to experts, the elderly, people with high blood pressure, diabetes, heart failure, especially those who are obese, need to see a doctor early when they have signs of high fever, pain in the lower back, painful urination, little urination, shortness of breath, and fatigue.
