Shortly after its formation, participating in the Central Highlands Campaign at the end of 1950, the Regiment successfully completed its mission of taking control of Xuan Trach, capturing 240 enemy soldiers and seizing all their weapons and equipment, thus beginning a glorious chapter in the history of the Hoai An Regiment. On January 14, 1951, the Regiment destroyed the Ba Huyen outpost, marking a significant leap forward in its organizational structure and combat tactics. During the Northwest Campaign (1951) and the Hoa Binh Campaign (1951–1952), the Regiment continued to demonstrate its unwavering resolve. Battalions undertook the task of encircling and attacking the Nghia Lo outpost, organizing defenses, ambushes, and eliminating many enemy forces, showcasing their coordination skills and courageous fighting spirit.
During the Northwest, Upper Laos, and Dien Bien Phu campaigns (1952-1954), in coordination with other units, Regiment 141 achieved many outstanding victories such as: destroying the 3rd Moroccan Battalion, liberating over 200,000 people in Upper Laos, participating in the attack on Him Lam Hill, E Hill, and Muong Thanh airfield... Many officers and soldiers bravely sacrificed their lives in combat, making a significant contribution to the final victory of the resistance war against French colonialism. Notable examples include martyrs and Heroes of the People's Armed Forces Phan Dinh Giot and Nguyen Van Thuan.

Following the 1954 Geneva Accords, the 141st Regiment participated in consolidating forces, rebuilding the North, supporting the Southern battlefield, and contributing to the construction of irrigation and industrial projects, thus building the North into a solid rear base. From 1964 onwards, the Regiment successively sent battalions to the Southern battlefield, achieving victories at Chop Chai, Van Tuong, Hoa Vang, Ba Na, and many other important battles.
From June 1973 to March 1975, the Regiment was tasked with holding and protecting the liberated area of Hoai An district. From then on, the Regiment was honored with the name "Hoai An Regiment" and continuously achieved outstanding victories in the Central Highlands Campaign and the historic Ho Chi Minh Campaign, contributing to the liberation of the South and the reunification of the country with achievements such as participating in the liberation of Dong Pho, Lai Nghi, Phu Cat airport, Quy Nhon, Phan Rang, and Cam Ranh; eliminating more than 12,277 enemy soldiers and seizing many weapons and equipment... On September 8, 1975, the Regiment and Battalion 7 were awarded the title of Hero of the People's Armed Forces by President Ton Duc Thang.

After the liberation of South Vietnam, Regiment 141 continued to participate in suppressing the Fulro rebels and defending the Cao Loc border (1978-1979), achieving many victories and leaving behind shining examples of unwavering fighting spirit. From 1985 to 1994, Regiment 141 was tasked with deploying forces to build a defensive posture on the Lang Giang border (Bac Giang province) and participating in defense along the Ha Tuyen front.
Since April 1994, the Regiment has been stationed in Dong Thuy village, Tan Thanh commune, Lang Son province; focusing on building the unit in the direction of "revolutionary, regular, elite, and progressively modern," improving the quality of training to be more practical, strengthening combat readiness and mobility; promoting the application of information technology and exploiting new weapons and equipment; while also paying attention to ensuring logistics and technical support, taking care of the soldiers' lives, and building a healthy and regular military cultural environment.

As a unit responsible for training, combat readiness, and carrying out other tasks assigned by the Division and Military Region, the Party Committee and command of Regiment 141 have consistently and thoroughly implemented resolutions and directives from higher authorities over the years, promptly determining leadership and guidance policies to organize the unit's activities effectively, with quality and depth. 100% of officers and soldiers possess strong political resolve, maintain peace of mind in their work, and have a clear understanding of their duties. Annually, inspections by the Ministry, Military Region, and Division regarding combat readiness and training have consistently assessed the Regiment as fully capable of meeting mission requirements in all situations. Over 75 years of building, fighting, and maturing, the Regiment and 5 subordinate units and individuals have been awarded the title of Hero of the People's Armed Forces. In 2010, on the 60th anniversary of its founding, the Regiment was honored to receive the Third-Class Order of National Defense from the President; for many consecutive years, it has been commended, awarded flags, and recognized as a "Unit of Excellent Training," a "Unit of Decisive Victory," and a "Unit of Comprehensive Strength" by the Prime Minister, the Ministry of National Defense, and the Command of Military Region 1. In 2024, Regiment 141 was honored to receive the flag of "Leading Unit in the Emulation Movement for Decisive Victory" from the Ministry of National Defense. Many collectives and individuals have been commended by the Military Region and the Division for their outstanding achievements in performing their duties.
Inheriting and developing the precious traditions built by generations of officers and soldiers throughout history, today, Regiment 141 continues to affirm its role as a core force in training and combat readiness; constantly improving overall quality, skills, mobility and combat capabilities; determined to maintain its position as a leading flag and a shining example of the 3rd Division, Military Region 1, worthy of the trust and affection of the Party, the Army and the people.
