On the afternoon of June 27th, Bach Mai Hospital held a discharge ceremony for patient Nguyen Quoc Trinh (19 years old, from Thanh Hoa province) after nearly 7 months of treatment. Notably, this is an exceptionally rare case, the first of its kind recorded in Vietnamese medical literature, and the 10th case worldwide to be successfully treated and discharged from the hospital.
Accordingly, Mr. Nguyen Quoc Trinh was diagnosed with paraneoplastic pemphigus (extramedullary hematopoietic tumor) and had a large retroperitoneal tumor measuring approximately 136x65x131mm. In December 2023, Mr. Trinh was brought to the Dermatology Department of Bach Mai Hospital by his relatives in a condition of severe ulceration of the oral mucosa, accompanied by skin lesions with manifestations of papules, purplish-red plaques, vesicles, and blisters scattered throughout the body...
The skin lesions rapidly progressed into extensive erosive patches, covering more than 70% of the body surface area. The painful, erosive lesions in the oral mucosa caused significant discomfort, leading to poor appetite and rapid physical deterioration. In addition to the severe skin and mucosal manifestations, the patient had a large retroperitoneal tumor compressing several surrounding organs.
Patient Trinh underwent multiple skin biopsies, but the results varied. It wasn't until January 2024 that the patient was diagnosed with paraneoplastic pempigus, in a state of severe debilitation.
"Pemphigus ectopia is a rare disease, not yet documented in Vietnamese medical literature. Worldwide, the incidence is 1 in 1 million people," shared Dr. Tran Thi Quyen from the Dermatology Department. She also noted that in the initial stages of treatment, the patient's lesions responded very poorly to conventional immunosuppressive therapies. The skin erosions continued to spread to over 80% of the body surface area, and the patient lost 10kg in a month, weighing only slightly over 30kg.
However, after nearly 200 days and two treatment cycles, patient Trinh was miraculously saved and returned to a normal life. According to Mr. Dao Xuan Co, Director of Bach Mai Hospital, diagnosing Pemphigus paraneoplastic syndrome was difficult, but treating the disease was an even greater challenge, requiring the coordination of many specialties and leading experts.
"The successful treatment of patient Trinh was thanks to the strengths of a comprehensive, top-tier general hospital like Bach Mai," Mr. Dao Xuan Co emphasized, expressing his gratitude to the Hanoi Social Insurance.
"The fact that the insurance agency covered the treatment costs for over six months, amounting to nearly 800 million VND, demonstrates the humanity and immense value of social insurance and health insurance. Patient Nguyen Quoc Trinh is from a poor household, orphaned at a young age, and his family is extremely poor. Without insurance coverage, it's unclear where his family would have managed to cover such a large treatment cost," the Director of Bach Mai Hospital expressed.
