The patient is TVK, 34 years old, residing in Con Dao. Two days before being admitted to the hospital, the patient had a dull headache in the right temple. At 7:30 am on October 17, the patient suddenly fell, had weakness in the left limbs, a crooked mouth and was taken to the Emergency Department of the Con Dao Military Medical Center.

Initially, the doctor determined that the patient had a stroke in the right hemisphere of the brain, during the first “golden hour”. Dr. Huynh Minh Triet, Department of Cerebrovascular Diseases, People’s Hospital 115, ordered a CT scan of the brain to assess the damage.
At about 8:10 a.m., while waiting for the scan, the patient's condition rapidly deteriorated: significant left-sided facial paralysis, difficulty speaking, almost no movement of the left arm, left leg weakness, and apathy on the left side of the body. The NIHSS score, used to assess the severity of a stroke, increased from 2 to 12 points - indicating a serious illness.
At 8:30 a.m., the results of the brain CT scan were urgently sent via the remote consultation system (PACS) to People's Hospital 115 for experts to review. The expert team, including Associate Professor, Dr. Nguyen Huy Thang, Head of the Department, and Master, Dr. Pham Nguyen Binh, Deputy Head of the Department, determined that there were no signs of bleeding in the brain, the ASPECTS score was 9, meeting the criteria for emergency intravenous thrombolytic therapy (rTPA) to dissolve the blood clot causing cerebral vascular occlusion.
Following the consultation, Dr. Huynh Minh Triet, in collaboration with Dr. Nguyen Thi Thanh Truc, Department of Anesthesia and Resuscitation, Binh Dan Hospital, performed thrombolytic treatment for the patient. Thrombolytic drugs were provided by People's Hospital 115 to Con Dao Medical Center.
During the infusion, the patient improved significantly, was alert, had good contact, left arm muscle strength 3/5, left leg 4/5, NIHSS score decreased from 12 to 6 points. At the end of the infusion, the patient almost completely recovered, only had mild left VII nerve palsy, left arm and leg muscle strength 5/5, NIHSS score decreased to 1 point.
Thus, the intravenous thrombolysis treatment for a patient with acute cerebral infarction at the Con Dao Military-Civilian Medical Center was a great success, demonstrating the effectiveness of the remote consultation system and timely professional coordination between People's Hospital 115 and the team of doctors working in rotation in Con Dao.
Ho Chi Minh City Department of Health has commended and highly appreciated People's Hospital 115 for successfully deploying the Stroke Unit right at Con Dao Military-Civilian Medical Center, marking a new task of the Specialist Doctor Rotation Program effectively deployed in Con Dao.
